IDT Care Manager Float Appleton (WI Based Work from Home)

Join our award-winning team at Lakeland Care as we support members across Wisconsin! We are seeking a Care Manager (CM) Float to serve frail elders, adults with physical disabilities, and adults with intellectual/developmental disabilities who are members of Lakeland Care (LCI). The team’s goal is to promote the overall well-being of our members through high-quality, person-centered, outcome-based care.

Position Summary: Under the general direction of the Care Management Supervisor, the CM Float provides care management and service coordination to LCI members. As a temporary team member, the CM arranges services based on comprehensive assessments, monitors service provision, and ensures compliance with policies and regulations. This role covers for CMs on extended leave and may require travel across multiple regions.

Responsibilities include:

  • Collaborating with RN Care Managers to coordinate care and follow-up services.
  • Developing and evaluating member-centered care plans.
  • Coordinating benefits and access to federal and state programs.
  • Conducting assessments and reassessments of member needs and risks.
  • Implementing risk mitigation to promote safety and independence.
  • Educating members on their rights and facilitating appeal processes.
  • Participating in ongoing training to ensure compliance and professional standards.
  • Maintaining accurate member records.
  • Participating in home visits and care conferences.
  • Maintaining confidentiality and compliance with HIPAA and other regulations.

Minimum Requirements:

  • Certified Social Worker in Wisconsin with at least 1 year of relevant experience, or
  • Bachelor’s degree in human services, psychology, or related field with 2 years of experience, or
  • Bachelor’s degree in another field with 3 years of experience working with the target populations.
  • Proficiency with computers and data entry.
  • Ability to access members’ homes (not required to meet ADA standards).
  • Ability to multitask in a fast-paced environment.
  • Ability to lift up to 25 lbs.
  • Valid driver’s license, acceptable driving record, and proof of insurance.
