Position Summary:

The Care Manager (CM) Float, as a temporary member of an interdisciplinary team (IDT), serves frail elders, adults with physical disabilities, and adults with intellectual/developmental disabilities who are members of Lakeland Care (LCI). The team’s goal is to promote the overall well-being of the members LCI serves by providing high-quality, person-centered, outcome-based care.

Under the general direction of the Care Management Supervisor, this Float position provides care management and service coordination to LCI members. As a temporary member of the IDT, the care manager arranges for the provision of services and supports based on a comprehensive assessment of the member's identified outcomes and needs. The IDT monitors the provision of services based on the member-centered plan per LCI policy and procedures, and Department of Health Services (DHS) contract requirements.

The CM Float position will provide coverage for CMs on extended leave, filling a gap between departures and new hires or other reasons deemed appropriate by the supervisor or regional manager.

This role may require coverage across multiple service regions and flexibility with travel.

Responsibilities & Competencies:
  1. Collaborate with the RNCM to coordinate acute and primary care services, care transitions, and follow-up care.
  2. Develop, coordinate, monitor, and evaluate outcome-based, member-centered plans, considering cost and effectiveness in authorizing services and selecting providers.
  3. Coordinate care and benefits to ensure comprehensive support and ongoing access to federal and state programs.
  4. Conduct comprehensive assessments of members’ outcomes, needs, and risks; perform reassessments as conditions change.
  5. Implement risk mitigation strategies to promote health, safety, and independence, respecting members’ rights.
  6. Educate members about their rights to appeal and grieve decisions; facilitate their participation in these processes.
  7. Participate in ongoing training; stay current with regulations, policies, and professional standards.
  8. Create and maintain member records as required by DHS and LCI policies.
  9. Coordinate and participate in home visits and care conferences to assess and reassess needs and coordinate interventions.
  10. Participate in team meetings and workgroups as required.
  11. Maintain confidentiality of member information and PHI in compliance with HIPAA and other regulations.
Requirements:
  1. Certified Social Worker in Wisconsin with at least 1 year of experience with Family Care populations, or
  2. Bachelor’s degree in human services, psychology, or related field with 2 years’ experience in similar populations, or
  3. Bachelor’s degree in any other field with a minimum of 3 years’ experience working with Family Care populations.
  4. Proficient with computers, data entry, and relevant software.
  5. Ability to access members’ homes, which are not ADA compliant.
  6. Ability to multitask in a fast-paced environment.
  7. Ability to lift up to 25 lbs.
  8. Valid driver’s license, acceptable driving record, and proof of insurance.
