Job summary

Socket.dev is seeking a Caseworker 2 to perform professional level social service work in the BH/IDS Agency. You will obtain social information, assist with benefit applications, and develop Individual Service Plans for consumers connected to Residential Day Programming, socialization, medical, fiscal and adaptive behavior goals.

Qualifications

• Requires a bachelor’s degree with relevant social science credits.
• Requires 2+ years in a social service aide role plus 2 years of college coursework.
• Equivalency options accepted with specified credit requirements.
• Includes 12 credits in sociology or related social sciences and two years of professional experience.

Responsibilities

• Obtain social information for development of treatment plans and enrollment.
• Assist consumers in filing SSI, DPW, and VA benefits applications.
• Develop Individual Service Plans detailing goals across multiple areas.
• Maintain files, records, and documentation for continuity of care.
• Act as advocate to ensure appropriate treatment approval.
• Coordinate and direct meetings to assess progress.
• Access supportive services for IDS clients in the community.
• Attend resident reviews to ensure service levels and program implementation.
• Provide crisis intervention and emergency services as coordinated by a supervisor.
• Provide technical assistance to service providers for compliance with regulations.

Education

Bachelor’s degree with 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science

Two years’ experience as County Social Service Aide 3 and two years of college coursework including 12 related credits

Equivalent combination of experience and training including 12 credits in related social services and one year as County Social Services Aide 3

Combination of 12 college credits in related social sciences and two years’ professional experience in developmental disabilities, mental health, counseling or social work
