Job summary

Socket.dev is seeking an entry-level Caseworker 1 to support Behavioral Health & Intellectual/Developmental Services with supervision. You will gather information for treatment plans, assist with benefit applications, and help develop individualized service plans for clients in the community.

Responsibilities include coordinating meetings, maintaining client records, and providing crisis intervention under supervision.

Qualifications

  • Must have exam on file with the State Civil Service Commission and must apply to the exam posting as directed.
  • Bachelor’s degree with 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science.
  • Two years’ experience as a County Social Service Aide 3 and two years of college coursework including at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social service.
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service and one year of experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions.
  • A combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or related social science and two years’ professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work or health and rehabilitative services.

Responsibilities

  • Obtain social information for development of treatment plans and enrollment into Behavioral Health & Intellectual/Developmental Services under supervision.
  • Assist consumers in filing applications for SSI, DPW and VA benefits.
  • Develop Individual Service Plans detailing goals in residential day programming, socialization, medical, fiscal and adaptive behavior.
  • Monitor Residential and Day Program providers to ensure implementation of services.
  • Maintain files, records and documentation for continuity of care and life management planning.
  • Coordinate and direct meetings to assess consumers’ progress in charted areas.
  • Access supportive community services including budgeting, shopping, leisure activities, social skills, personal hygiene, and individual counseling.
  • Attend resident reviews to ensure adequate service levels and program implementation.
  • Provide crisis intervention and emergency services as coordinated by supervisor.
  • Provide technical assistance to service providers to ensure compliance with regulations.

Responsibilities of this position include the following:

Caseworker 1 is entry level social service work in the BH/IDS Agency and is highly supervised.

  • Obtain social information for the intellectually and developmentally delayed for the development of treatment plans and enrollment into Behavioral Health & Intellectual/Developmental Services under the supervision of the Supports Coordinator Supervisor.
  • Assist consumers in filing application for SSI, DPW and VA benefits.
  • Must participate in the development of Individual Service Plans detailing goals in the areas of: Residential Day Programming, socialization, medical, fiscal and adaptive behavior for each consumer. Monitor Residential and Day Program providers to assure implementation of stated Individual Program Plan services.
  • Responsible to maintain files, records and other documentation necessary for the continuity of consumer care and development of life management plans and act as an advocate for the consume to ensure the most appropriate treatment is approved.
  • Coordinate and direct meetings to assess the consumers’ progress in charted areas.
  • Access supportive services for the IDS clients residing in the community. These supportive services will include: budgeting, shopping, leisure time activities, social skills, personal hygiene, and individual counseling in making adjustment to community life.
  • Attend resident reviews at State Centers, Residential Programs, Vocational Rehabilitation Facilities and other service providers to ensure adequate levels of service and implementation of programs designed for each client.
  • Provide crisis intervention and emergency services as coordinated by the supervisor of said services.
  • Provide technical assistance to service providers in maintaining compliance with 2176 Community Waiver Regulations and Person Family Direct Supports Waiver.
  • Responsible for all other position related duties not specified above.

Must meet the required minimum experience and training for this job:
