Job summary

Qualifications

Pharmacist by licensure in California or eligible for licensure
Experience in clinical pharmacy services and patient counseling preferred
Experience working with interdisciplinary teams in a hospital setting

Responsibilities

Provide pharmaceutical care to patients in IDS and related services
Obtain medication histories and review medication profiles
Educate patients on proper use of medications and potential interactions
Collaborate with clinicians on drug choices and therapeutic plans
Assist in oversight of pharmacy technicians and management of investigational medications
