Job description

The Department of Pharmaceutical Services provides comprehensive medication services across the UCSF Health System, supporting patient care and clinical pharmacy functions.

Responsibilities

• Obtain and review medication histories and profiles to ensure safe and effective therapy.
• Provide accurate drug information to patients and healthcare providers.
• Collaborate with clinicians on drug selection and therapeutic plans.
• Ensure patients have access to required medications and understand their proper use.
• Educate patients about cautions, adverse reactions, and drug interactions.
• Oversee pharmacy technicians in tasks such as insurance authorization, prescription preparation, and dispensing.
• Manage clinical activities and deliver pharmaceutical services within inpatient and other acute care settings.
• Participate as a member of an interprofessional team, including physicians, advanced practice providers, and nurses.
• Maintain flexibility to work at various UCSF Health locations as operational needs arise.

Required Qualifications

• Doctor of Pharmacy (PharmD) degree or equivalent, as determined by the National Board of Pharmacy.
• Current pharmacy license in good standing in the State of California.
• Basic Life Support (BLS) certification from the American Heart Association.

Preferred Qualifications

• Completion of an ASHP-accredited PGY1 Pharmacy Residency or at least 3 years of inpatient/operational pharmacy experience.
• Advanced practice board certification (e.g., Board of Pharmacy Specialty certifications, AAHIVP, etc.).
