Job summary

Lakes Regional is seeking an IDD Provider Specialist to plan and deliver group and individualized activities for individuals with developmental disabilities in ISS and residential settings, while providing direct care services. You will implement therapeutic programs, document services daily, and collaborate with care coordinators, families, and the interdisciplinary team.

The role includes assisting with daily living activities, medications, transportation, and health needs.

Qualifications

• High School diploma or equivalent required.
• Six months of work experience preferred.
• Knowledge of therapeutic techniques for individuals with IDD preferred.

Responsibilities

• Plan and implement group and individualized activities.
• Liaise with care coordinators and families.
• Complete daily documentation of services per procedures.
• Serve on interdisciplinary team as needed.
• Provide direct care including daily living activities and health support.
• Assist with medications, transportation and health-related needs.
• Implement behavior management programs and safety procedures.
• Maintain a safe, clean, and supportive environment.
