Job summary

Lakes Regional Community Center is seeking an IDD Provider Specialist to lead group and individualized activities within ISS and residential settings. The role includes direct care, medication administration, daily documentation, and collaboration with care coordinators and family members to support individuals with developmental disabilities.

Responsibilities include implementing therapeutic programs, coordinating with the interdisciplinary team, and ensuring a safe, supportive environment.

Qualifications

  • Must have six months of any type of work experience.
  • Knowledgeable of therapeutic techniques and modalities when working with individuals with intellectual and developmental disabilities.
  • Ability to exercise independent judgment and work with minimal supervision.
  • Effective communication with individuals and co-workers.
  • MS Word and Excel familiarity preferred.

Responsibilities

  • Implement and evaluate therapeutic programs and plan individual activities.
  • Liaise with Provider Care Coordinators and family members.
  • Daily documentation of services, adhering to safety procedures and assisting with facility/vehicle maintenance.
  • Serve as a member of the interdisciplinary team.
  • Provide direct care including assistance with daily living activities and medications as needed.
  • Provide training/assistance with activities of daily living; housekeeping and transportation support.
  • Implement behavior management programs and emergency interventions when necessary.
  • Maintain a productive, clean, safe environment and be a positive role model.

Description The IDD Provider Specialist position is responsible for group and individualized activities at the Individualized Skills and Socialization (ISS) and/or residential settings program, and also provides direct care services to persons with developmental disabilities. Responsibilities include:

  • Implementation and evaluation of therapeutic programs for individuals with intellectual and developmental disabilities, planning individual activities
  • Liaison with individual's Provider Care Coordinators, and family members.
  • Daily completion of documentation of services, complying with safety procedures and assist with monitoring facility and vehicle maintenance.
  • Serves as a member of the interdisciplinary team, as requested.
  • Provides direct care services to include assistance and training with personal hygiene, food preparation, feeding, mobility, community access, recreation, leisure activities, and maintenance of basic health care needs.
  • Provision of training and assistance with all activities of daily living, including administration and monitoring of medications, housekeeping, transportation, and other individualized goals.
  • Implements behavior management programs, and provides emergency behavior or health-related interventions, as necessary.
  • Fosters a productive, clean, comfortable, and safe environment for individuals; and serves as a positive role model for those we serve.

  • Exercise independent judgment, be able to work with minimal supervision and communicate effectively with individuals and co-workers.
  • Prefer working computer skills including MS Word and Excel.
Work Hours:

Multiple schedules available for PRN (as needed) including but not limited to Overnights, Weekends, and Monday - Friday.
