Responsibilities

Coordinate the development and implementation of client‑based habilitative programming, including individual habilitation plans (IHP), individual program plans (IPP), and individual behavior guidelines.

Act as chairperson for inter‑disciplinary team meetings, ensuring comprehensive delivery of services—such as speech, medical, pharmacy, social, psychology, psychiatry, vocational, and educational services—to a caseload of 8–30 clients.

Supervise assigned staff, which may include residential care supervisors, hospital aide coordinators, therapeutic program workers, teachers, activity therapy specialists, juvenile correctional officers, and social workers.

Provide training in effective delivery of training techniques/methods, ensure habilitative services and documentation meet Medicaid ICF/IID requirements, and maintain accurate records of program implementation.

Conduct periodic rounds of treatment locations—cottages, modules, units, buildings—to verify proper implementation of programming, monitor staff documentation, confirm client interactions, and ensure compliance with Medicaid regulations.

Chair team meetings to review, analyze, and modify 30/90‑day and annual reviews of client habilitation plans, facilitate discussion, develop client goals, and correct deficiencies; participate in mock facility surveys and recommend policy changes.

Coordinate client home visits, transfers (inter‑ or intra‑facility), and discharges for efficiency; act as relief residential care supervisor or assist therapeutic program workers in client training; attend meetings, trainings, workshops, and seminars; serve on committees and task forces; liaise with client families, guardians, case‑management staff, and community staff; prepare related reports, forms, and correspondence.

Qualifications

• Minimum of one year of experience working directly with individuals with intellectual or developmental disabilities.
• One of the following credentials:
  • Doctor of Medicine or Osteopathy
  • Registered Nurse
  • Bachelor’s degree in a professional human services field (e.g., Psychology, Education, Anthropology)
