Job summary

RHA Health Services, LLC is seeking an LPN to provide holistic nursing care for individuals with intellectual and developmental disabilities in an office-based setting in Chattanooga, TN. The role involves routine visits to group homes and coordinating medical care with an interdisciplinary team.

Ideal candidates have an active LPN license, CPR certification, and experience with IDD or case management. Some travel between homes is required, with regular 8:30am–5:00pm hours and a Monday–Friday

Qualifications

• Graduation from an accredited licensed nurse program.
• Current LPN license with no restrictions to practice nursing.
• Current CPR Certification or eligibility to certify for CPR.
• Possess credentials required by Federal or State law.
• Must communicate effectively with Interdisciplinary Team Members and families.
• Must be able to work independently, be organized, and prioritize tasks.
• Must be at least 18 years of age.

Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities.
• Observe, assist with gathering health data and report health needs to the RN.
• Coordinate preventative and routine medical care for people supported.
• Document nursing care provided.
• Assist physicians with on-site medical clinics when applicable.
• Provide health trainings to unlicensed personnel per state regulations.
• Conduct medication pass observations to ensure staff competency.
• Perform quality assurance checks to ensure unlicensed personnel perform health tasks as assigned.
• Observe for abnormal health findings and report to physician.
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team.
• Advocate for people supported and assist with community supports to promote health.
