Job summary

00 RHA Health Services, LLC in Chattanooga, TN is hiring an LPN (IDD LPN) for day shifts in group home settings. This on-site, full-time role runs 8am–8pm with every other weekend, offering a $28 per hour rate.

You will provide holistic nursing care to individuals with IDD, coordinate routine medical care, document care, and support on-site clinics. A current LPN license, CPR certification, and ability to work with an interdisciplinary team are required.

Qualifications

• Graduation from an accredited licensed nurse program.
• Current LPN license with no restrictions to practice nursing.
• Current CPR Certification or eligibility to certify for CPR.
• Possess credentials required by Federal or State law.
• Must communicate effectively with the Interdisciplinary Team and families.
• Must be able to work independently, be organized, and prioritize tasks.
• Must be at least 18 years old.

Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities.
• Observe, collect health data, and report health needs to the RN.
• Coordinate preventive and routine medical care for people supported.
• Document nursing care provided.
• Assist physicians with on-site medical clinics where applicable.
• Provide health-related trainings to unlicensed personnel per state regulations.
• Conduct medication pass observations to ensure staff competency.
• Conduct medication closest assessments.
• Perform quality assurance checks to ensure unlicensed staff perform health tasks.
• Observe for and report abnormal health findings to the physician.
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team.
• Advocate for people supported and help develop community supports.
