RHA Health Services, LLC is hiring an IDD Facility Nurse (RN) for an on-site PRN role at Bear Creek ICF in La Grange, NC. The RN/LPN will oversee health care, monitor individuals, and provide training for staff to promote health and independence.

The role requires an active nursing license and strong communication with the IDT, with compensation ranging from $30 to $38 per hour depending on experience. Comprehensive benefits are available for eligible employees.

Qualifications

• Graduate of an accredited LPN or RN program.
• Active, unrestricted nursing license.
• Strong communication with interdisciplinary team and families.
• Ability to work independently, stay organized, and prioritize tasks.
• CPR certification or willingness to obtain certification.

Responsibilities

• Provide holistic nursing care to individuals with IDD.
• Observe, assess, and evaluate health needs of individuals.
• Deliver health-related training to unlicensed personnel.
• Educate individuals on health monitoring and medication administration training.
• Administer medications and treatments or support self-administration.
• Schedule and maintain preventative medical appointments.
• Conduct medication closet assessments.
• Perform quality assurance checks on health tasks by unlicensed personnel.
• Collect and document health data.
• Conduct comprehensive nursing assessments.
• Monitor and report abnormal findings to physicians promptly.
• Collaborate with physicians, pharmacies, and IDT.
• Advocate for individuals and develop community supports.

Position Overview

As an LPN or RN with RHA, you will have the opportunity to practice holistic nursing while making a meaningful impact in the lives of others. In this role, you will provide healthcare oversight and monitoring for the individuals we support, promoting their overall health, independence, and quality of life.

Key Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities (IDD)
• Observe, assess, and evaluate the health needs of the individuals we support
• Deliver health-related training to unlicensed personnel
• Educate individuals on how to monitor their own health and, when appropriate, provide medication administration training
• Administer medications and treatments, or assist individuals with self-administration
• Ensure preventative and routine medical appointments are scheduled and maintained
• Conduct medication closet assessments
• Perform quality assurance checks to ensure unlicensed personnel are completing assigned health-related tasks accurately
• Collect and document health-related data
• Conduct comprehensive nursing assessments
• Monitor for and report abnormal findings to physicians in a timely manner
• Collaborate and communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team (IDT)
• Advocate for individuals and assist in developing community supports to promote optimal health and independence

Qualifications

• Graduate of an accredited License Practical Nursing (LPN) OR Registered Nurse (RN) program
• Active, unrestricted nursing license
• Strong communication skills with interdisciplinary team members, individuals served, and their families
• Ability to work independently, stay organized, and effectively prioritize tasks
• Current CPR certification or willingness to obtain certification
