Job summary

The Arc of Central Alabama seeks a Connected Living Coordinator to lead day-to-day operations of the CLP, ensuring individualized supports for people with IDD living in host-family homes under the ADMH Community Waiver Program. You will recruit, train, and support Host Family Caregivers while maintaining PCPs and monitoring quality and compliance.

Qualifications

  • Minimum Bachelor’s degree in human services or related field and QDDP eligibility
  • At least 1 year direct experience with IDD per Title XIX guidelines
  • Completion of ADMH QDDP training or ability to complete on schedule
  • Must pass background check and drug screen; valid driver’s license
  • Travel to host homes and program sites required

Responsibilities

  • Recruit, screen, and select Host Family Caregivers and match individuals to host homes
  • Verify qualifications, training, and background checks for all household members 18+
  • Develop and update Person-Centered Plans (PCP) with care teams
  • Conduct regular in-home monitoring visits and ensure HCBS compliance
  • Support certification processes and documentation for AFH settings
  • Prepare for HCBS compliance reviews and coordinate remediation actions
  • Train and supervise Host Family Caregivers on PCP implementation and safety
  • Review service documentation for completeness, accuracy, and timeliness
  • Monitor resident health and safety; respond to risks and incidents
  • Serve as primary contact for families and support coordinators
  • Lead interdisciplinary meetings and track action items to completion
  • Monitor billing/utilization data and flag discrepancies
