Job summary

The Arc of Central Alabama is seeking a Connected Living Coordinator to lead the day-to-day operation of the CLP program, a community living option under the ADMH Community Waiver Program. You will recruit, train, credential, and support host-family caregivers, develop and monitor Person-Centered Plans, and ensure HCBS compliance and Title XIX standards.

Qualifications

  • Bachelor's degree in a human services field meeting QDDP requirements.
  • Minimum of one year direct experience working with individuals with IDD per Title XIX, 42 CFR 483.430.
  • Completion of ADMH QDDP (Relias) training, or ability to complete within required timelines.
  • Ability to complete ADMH Prospective Provider Orientation and AFH training, and support ACAP certification.
  • Must pass statewide/criminal background check and pre-employment drug screen.
  • Valid Alabama driver's license and ability to travel to host homes and program sites.

Responsibilities

  • Recruit, screen, and select Host Family Caregivers and coordinate matching based on individual needs.
  • Verify host-family qualifications and training records; confirm background checks for all adults.
  • Develop and update Person-Centered Plans in collaboration with individuals and teams.
  • Conduct regular in-home monitoring visits to AFH to ensure PCP compliance and needs are met.
  • Support AFH approval, certification processes, and related documentation.
  • Prepare for HCBS compliance reviews and coordinate remediation actions with timelines.
  • Train and supervise Host Family Caregivers on PCP implementation and safety.
  • Review service documentation for completeness, accuracy, and timeliness.
  • Monitor health, safety, and rights, responding to illness or risk appropriately.
  • Communicate with families, guardians, host families, and coordinators about placement and services.
  • Lead or participate in team meetings and periodic plan reviews.
