Job summary

AbleLight in Bloomington, MN is seeking a Case Manager to deliver person-centered care in residential, supported, or shared living settings. You will help individuals with developmental disabilities plan, access necessary services, and work toward their full potential.

You will facilitate assessments, develop plans, and connect clients with medical, educational, and social resources, while ensuring timely documentation and collaboration with interdisciplinary teams.

Qualifications

• Bachelor’s degree in related field or commensurate experiences.
• Four years of case management experience.
• Possession of a valid driver's license and good driving record, as defined by AbleLight.
• Must be 18 years of age, or older.

Responsibilities

• Facilitates planning and implementation of person-centered plans using assessments and progress reviews.
• Assists adults with Intellectual Disabilities to access needed medical, social, educational services.
• Provides or coordinates direct case management activities for adults with Intellectual Disabilities.
• Conducts needs assessments and writes comprehensive assessments.
• Supports plan of care development, review, and timely implementation.
• Identifies and links needed services/resources per Plan of Care.
• Expands social support networks and connects with agencies.
• Reassesses progress quarterly and documents outcomes.
• Monitors service adequacy and maintains records for billing.
• Attends interdisciplinary and interagency meetings.
• Completes required documentation for case management records.
