Job summary

Core Health Provider Services, Inc. is seeking an LPN in Marietta, GA to provide direct nursing care for adults with intellectual and developmental disabilities.

You will administer medications, monitor health, support behavioral health needs, and collaborate with a multidisciplinary team in accordance with DBHDD standards. Responsibilities include documenting health changes, participating in ISP meetings, and educating individuals and caregivers on health maintenance and medication adherence.

Qualifications

• At least 1 year of nursing experience.
• Graduate of an approved professional nursing program.
• Current Georgia LPN license without restrictions.
• CPR/First Aid certification (current).
• Valid driver’s license and car insurance.
• Ability to pass a criminal background check.
• TB test and health clearance.

Responsibilities

• Administer prescribed medications and treatments.
• Communicate with RN, DDP, and interdisciplinary team as needed.
• Participate in team meetings and health reviews.
• Document communication with physicians and family.
• Monitor and document changes in health and behavior.
• Record vital signs and health status.
• Assist with personal hygiene and daily living activities.
• Collaborate with case managers and physicians.
• Participate in ISP meetings and implement care plans.
• Maintain compliant documentation per DBHDD protocols.
• Respond to emergencies and adverse reactions appropriately.
• Educate individuals and caregivers on health maintenance and medication adherence.
