Qualifikationen

• Requires a four-year degree in a human services field and one year of experience working with persons with intellectual disability OR equivalent experience.
• QDDP certification or eligibility is required.

Aufgaben

• Provide direct case management services to individuals and families based on thorough assessments.
• Maintain caseload (28+) of Medicaid State Plan Option and waiver consumers and non-Medicaid consumers.
• Conduct initial intakes and routine assessments to determine interests, strengths, needs and progress.
• Develop service plans that are clear, quantitative, and individualized.
• Meet with clients as scheduled by state/division/department.
• Enter consumer data into the department’s electronic health record accurately and on time.
• Ensure plans and documentation meet CQI, Medicaid, and licensure standards.
• Maintain consumer charts per policy and regulatory standards.
• Respond effectively to crisis or emergency situations with supervisory support.
