On-site shifts include weekend commitments in a hospital setting. Qualified candidates hold an RN license in Massachusetts with 2+ years of Med/Surg experience; ADN is required, BSN strongly preferred.

Qualifications

• RN Case Manager experience required.
• Licensed RN in the state of Massachusetts.
• Minimum of 2 years Med/Surg experience required.
• Case Manager certificate preferred.

Responsibilities

• Coordinate patient care across the continuum.
• Assess, plan, implement, and evaluate patient care needs.
• Collaborate with healthcare team to improve patient experience and outcomes.
