Responsibilities

• Assess and analyze health status of patients in assigned unit.
• Perform and document initial and ongoing patient assessments.
• Coordinate discharge planning and plan patient care with the team.
• Develop and update individualized care plans within policy timelines.
• Direct and coordinate nurse and care team activities to ensure quality care.
• Communicate care plans during shifts and in handoffs.
• Transcribe and implement physician orders; document patient responses.
• Provide patient education and evaluate understanding.
