Qualifications

Responsibilities

• Assess and document patient status and care needs upon admission or point of care.
• Organize health data for accurate, complete access.
• Identify problems needing immediate attention and plan care.
• Document diagnoses and plan of care with measurable goals.
• Coordinate discharge planning with patient/family and team.
• Provide patient and family education based on evidence-based practice.
• Communicate with the care team regarding patient progress and outcomes.
• Contribute to multidisciplinary meetings and referrals as needed.
