SSM Health St. Joseph Hospital – St. Charles is seeking a registered pharmacist to provide clinical pharmacy services in a fast-paced hospital setting. You will review patient drug histories, make therapy recommendations, and collaborate with physicians and nurses to optimize patient care.

This role requires a Pharm.D. or equivalent, two years of experience or a PGY1 residency, and board certification in pharmacotherapy or similar. The position is full-time, day shift, onsite in St.

Qualifications

• Pharmacy degree from an accredited school.
• Two years experience or completion of PGY1 residency.

Responsabilités

• Provides pharmaceutical care in their specialty practice area including review of patient drug history and drug therapy recommendations.
• Plans and measures clinical processes to improve outcomes and coordinate DUEs.
• Coaches and trains staff, residents, students and other providers.
• Promotes interdisciplinary teamwork with clinical pathways and medication safety.
• Performs other duties as assigned.
