Centerpoint Medical Center is seeking a Pharmacist Clinical Specialist to provide advanced clinical pharmacy services. The role includes monitoring patient regimens, interpreting orders, and supporting pharmacy operations within a hospital setting.

The ideal candidate will hold a PharmD or BS in Pharmacy, with MTM experience and completion of PGY1, plus BCPS or related board certification. ICU/critical care experience is highly valued.

Qualifications

• Pharmacist with Medication Therapy Management experience.
• BS or PharmD from an ACPE-accredited school of pharmacy; PharmD preferred.
• Completion of PGY1 residency or hospital experience required; PGY2 preferred.
• Board certification in pharmacotherapy (BCPS) or relevant specialty board.
• 5 years of experience required; ICU/critical care experience preferred.

Responsabilités

• Interpret orders and verify them in the pharmacy system.
• Accurately compound and dispense pharmaceuticals.
• Monitor drug therapy for interactions and dosing appropriateness.
• Provide clinical consultation and cost-effective therapeutic alternatives.
• Participate in rounds and pharmacy improvements and education.
