Responsibilities

• Assess and analyze health status of patients on the unit.
• Perform and document comprehensive assessments and ongoing assessments.
• Develop and update individualized care plans with the health care team.
• Coordinate patient care activities and communicate plans to the team.
• Provide patient education and document outcomes.
• Participate in Performance Improvement activities and Core Measures.
• Maintain licensure and certifications as required.
