Job summary

UMass Memorial Health is seeking a clinical pharmacist to deliver comprehensive medication management, integrating clinical services with education and research. The role focuses on improving patient safety and clinical outcomes within a large health system in Central Massachusetts.

The position supports medication order management, patient and staff education, and ongoing clinical review under supervision. A Pharm.D.

Qualifications

• Licensed pharmacist with Pharm.D. or BSPharm and hospital experience preferred.
• Massachusetts licensure required or eligible for licensure.
• Clinically oriented pharmacist with strong communication and teaching skills.

Responsibilities

• Provide comprehensive medication management to improve patient outcomes.
• Review medication histories, educate patients, develop and monitor therapeutic plans.
• Counsel patients on discharge medications and ensure smooth transitions of care.
• Oversee dispensing processes and supervise pharmacy technicians to ensure safety and accuracy.
• Participate in regulatory activities and quality improvement initiatives.
