Qualifications

• Experience: One year as a registered nurse.

Responsibilities

• Collects data and completes nursing assessments using age-specific standards and tools.
• Develops an individualized plan of care with patient/family, provider and interdisciplinary team.
• Updates the plan based on changes in patient condition and communicates changes to the team.
• Identifies goals and outcomes to be attained prior to discharge and validates with patient/family.
• Delivers direct care per established nursing plan and delegates tasks as appropriate.
• Monitors quality of care and safety; ensures appropriate surveillance of patients.
• Maintains documentation and communications to support patient care needs and continuity.
