Job summary

Easterseals New Jersey is seeking a Case Manager to coordinate Integrated Case Management Services (ICMS) for consumers in Somerset County, NJ. The role emphasizes linking individuals to adequate services and resources to improve quality of life.

Responsibilities include home visits, crisis assessment, discharge planning, and collaboration with multidisciplinary teams, while maintaining compassionate, client-focused care and ensuring transportation to appointments.

Qualifications

• Master’s in Social Work/Counseling/Psychology or related field; or BA in behavioral health with 1+ year experience.
• Budget management, EHR proficiency, strong verbal and written communication.
• Ability to manage multiple priorities in a fast-paced, compassionate manner.
• Requires driving; valid license and post-offer driving eligibility verification.

• Provide ongoing support to consumers in Somerset County to help independence and access mental health services.
• Conduct home visits to engage consumers to reduce hospitalization risk and promote community integration.
• Assist consumers in accessing needed services and ensure transportation to appointments.
• Develop individualized service plans and ensure engagement with community services.
• Assess need for crisis intervention and assist in psychiatric emergencies.
• Encourage wellness and recovery while maintaining dignity and respect.
• Work with multi-disciplinary teams to develop discharge plans and ensure continuity of care.

Its services for individuals with mental health conditions provide daily living support and community-based programs that teach valuable life skills through instruction and real-world experiences.

What You’ll Do:

The Case Manager coordinates and manages the care of consumers in Integrated Case Management Services (ICMS). ICMS provides linkages to help individuals manage their illness and secure necessary services and resources to improve their quality of life.

• Through community outreach within Somerset County, NJ, provide ongoing support to consumers to help them function independently and access mental health services.
• Conduct home visits to engage with consumers in their environments to reduce risks of hospitalization and promote functional integration into the community.
• Assist consumers in accessing needed services and ensure transportation to appointments.
• Develop individualized service plans and ensure the consumer’s engagement with community mental health and non-mental health services.
• Assess the need for crisis intervention and assist in psychiatric emergency situations.
• Encourage wellness and recovery while maintaining individual dignity and respect.
• Work with multi-disciplinary teams to develop discharge plans and ensure continuity of care.

Who You Are:

• Education & Experience: Master’s degree in Social Work, Counseling, Psychology, or a related field; OR a Bachelor’s degree in a behavioral health-related field with at least one year of relevant experience.
• Skills: Skilled in budget management, proficient with technology including Electronic Health Records, and equipped with strong verbal and written communication abilities.
• Attributes: Able to manage multiple priorities in a fast‑paced, deadline‑driven environment while maintaining a compassionate, flexible, and client‑focused approach to care.
• This role requires driving as an essential function. Candidates must have a valid driver's license and be subject to post-offer verification of driving eligibility consistent with company insurance requirements.

  • Bachelor’s Degree: $44,000 – $48,000 annually
  • Master’s Degree: $48,000 – $52,000 annually
    • Final salary is based on education, experience, and internal equity
