Job summary

Easterseals New Jersey is seeking a dedicated Case Manager in Hunterdon County, NJ to coordinate the care of consumers in Integrated Case Management Services. This position involves providing ongoing support to help individuals access necessary mental health services while ensuring continuity of care through engagement in their environment.

Work Schedule: Full-Time: Monday-Friday (8:30am-4:30pm or 9:00am-5:00pm)

What You’ll Do

• The Case Manager coordinates and manages the care of consumers in Integrated Case Management Services (ICMS). ICMS provides linkages to help individuals manage their illness and secure necessary services and resources to improve their quality of life.
• Through community outreach within Somerset County, NJ, provide ongoing support to consumers to help them function independently and access mental health services.
• Conduct home visits to engage with consumers in their environments to reduce risks of hospitalization and promote functional integration into the community.
• Assist consumers in accessing needed services and ensure transportation to appointments.
• Develop individualized service plans and ensure the consumer’s engagement with community mental health and non-mental health services.
• Assess the need for crisis intervention and assist in psychiatric emergency situations.
• Encourage wellness and recovery while maintaining individual dignity and respect.
• Work with multidisciplinary teams to develop discharge plans and ensure continuity of care.

Who You Are

• Education & Experience: Master’s degree in social work, counseling, psychology or related field from an accredited institution and one (1) year of relevant supervisory experience; OR Bachelor of Arts in social work, counseling, psychology or related field from an accredited institution, three (3) years of post‑graduate work experience in mental health and one (1) year of relevant supervisory experience.
• Skills: Skilled in budget management, proficient with technology including Electronic Health Records, and equipped with strong verbal and written communication abilities.
• Attributes: Able to manage multiple priorities in a fast‑paced, deadline‑driven environment while maintaining a compassionate, flexible, and client‑focused approach to care.
• Driving Requirement: This role requires driving as an essential function. Candidates must have a valid driver’s license and be subject to post‑offer verification of driving eligibility consistent with company insurance requirements.

Compensation

• Bachelor’s Degree: $44,000 – $48,000 annually
• Master’s Degree: $48,000 – $52,000 annually
• Final salary is based on education, experience, and internal equity.
