Job summary

Easterseals New Jersey is seeking a full‑time Case Manager to coordinate Integrated Case Management Services (ICMS) for individuals with mental health needs in Somerset County, NJ, with support across a calendar‑based weekday schedule.

Responsibilities include home visits, linkage to services, transportation facilitation, and developing individualized plans to promote recovery and community participation. Driving is required; a valid license is essential.

Qualifications

• Master’s degree in Social Work, Counseling, Psychology, or a related field; OR a Bachelor’s degree in a behavioral health-related field with at least one year of relevant experience.
• Skilled in budget management, proficient with technology including Electronic Health Records, and equipped with strong verbal and written communication abilities.
• Able to manage multiple priorities in a fast‑paced, deadline‑driven environment while maintaining a compassionate, flexible, and client‑focused approach to care.
• Driving is an essential function; candidates must have a valid driver’s license and be subject to post‑offer verification of driving eligibility.

What You’ll Do:

The Case Manager coordinates and manages the care of consumers in Integrated Case Management Services (ICMS). ICMS provides linkages to help individuals manage their illness and secure necessary services and resources to improve their quality of life.

• Through community outreach within Somerset County, NJ, provide ongoing support to consumers to help them function independently and access mental health services.

• Assist consumers in accessing needed services and ensure transportation to appointments.
• Develop individualized service plans and ensure the consumer’s engagement with community mental health and non-mental health services.
• Assess the need for crisis intervention and assist in psychiatric emergency situations.
• Encourage wellness and recovery while maintaining individual dignity and respect.
• Work with multi-disciplinary teams to develop discharge plans and ensure continuity of care.

Who You Are:

• Education & Experience: Master’s degree in Social Work, Counseling, Psychology, or a related field; OR a Bachelor’s degree in a behavioral health-related field with at least one year of relevant experience.
