Job summary

Linc Housing is looking for a full-time Case Manager for their Intensive Case Management Services in Inglewood, CA. The Case Manager will assist formerly homeless individuals with chronic medical conditions and co-occurring disorders by providing strength-based case management and service coordination.

Key responsibilities include working with community agencies, assisting with paperwork, and conducting assessments. Ideal candidates will have relevant case management experience, strong communication skills, and the ability to work independently as well as part of a team.

POSITION TITLE: Intensive Case Management Program: Case Manager

FLSA STATUS: Non-exempt

Property Focus: This position works with people who have experienced homelessness. It is a full‑time position working in Inglewood.

All staff who work directly with residents undergo background checks and fingerprinting for safety.

Hiring Range: $23.69 to $27.87 per hour. New hires are placed within the hiring range based on qualifications, experience, and internal equity.

Position Summary: The Case Manager will be part of the Intensive Case Management Services team that works with formerly homeless individuals with chronic medical conditions and co‑occurring disorders. The role reports directly to the Senior Case Manager of ICMS and coordinates services using best practices such as motivational interviewing, harm reduction, critical time intervention, and stages of change. The primary function is to mitigate the risk of individuals losing their housing by enhancing problem‑solving, coping skills, and self‑coordination. The Case Manager collaborates with families, property management, housing locators, third‑party service providers, and other Linc staff.

Essential Functions:

  • Work closely with referring agencies such as the VA and local Housing Authorities.
  • Assist with paperwork required for lease‑up processes.
  • Collaborate with Linc staff, VA staff, and property managers.
  • Develop and maintain rapport with community and establish connections to mental health, substance abuse, employment, health, housing retention, and other services.
  • Provide strength‑based case management and service coordination to help reach treatment plan goals.
  • Manage and facilitate community collaboration.
  • Formulate intervention plans for residents and proactively adjust care plans as needed.
  • Maintain knowledge of community agencies and procedures.
  • Provide programs and support groups that promote physical and mental health, social development, and academic performance for residents of all ages.
  • Offer interventions and crisis management services as needed.
  • Conduct individual, group, assessment, support sessions, and residential programs for all residents.
  • Attend scheduled team meetings and trainings.
  • Report abuses as a mandated reporter.
  • Attend appointments with clients monthly and as required.
  • Maintain and submit time sheets, mileage logs, data reports, and other documentation accurately and timely.
  • Provide coverage for occasional programs during evenings and weekends.

Knowledge, Skills & Abilities:

  • Well‑developed psychosocial assessment and intervention skills.
  • Basic computer knowledge (MS Word, Excel); PowerPoint preferred.
  • Flexibility in scheduling and prioritizing tasks.
  • Critical thinking and good judgment in decision‑making, organizational and record‑keeping skills.
  • Ability to implement housing‑first principles with motivational interviewing, critical time intervention, harm reduction, and trauma‑informed care strategies.
  • Experience with Permanent Supportive Housing models.

Skill Level:

  • Superior writing and communication skills.
  • Understanding of HMIS (Homeless Management Information System).
  • Proficient in personal computer use.
  • Expertise in intake, assessment, and service planning.
  • Proficiency in documentation, including progress notes for every contact.
  • Experience with HIPAA policies and practices.
  • Excellent interpersonal skills for working with diverse community organizations and residents.
  • Team collaboration capabilities.

Abilities:

  • Implement and manage day‑to‑day intensive case management tasks.
  • Attend local client appointments as required.
  • Work with single individuals and families.
  • Ensure treatment plan goals are met or updated.
  • Manage multiple priorities in a fast‑paced environment.
  • Provide services dignifiedly and non‑judgmentally.
  • Work independently and as part of a team.
  • Facilitate life skills and support groups.
  • Communicate via email, telephone, and in writing regularly.
  • Work a flexible schedule, including evenings, weekends, and travel up to 50% of the time.

Education and Experience Requirements:

  • Relevant case management experience or experience with mental health services, substance use/co‑occurring disorders, or vulnerable populations providing intensive case management; experience with veterans and families is a plus.
  • Minimum of two years’ experience with homeless individuals, preferably with mental illness and/or substance addictions, or equivalent education/experience.
  • Effective communication with diverse residents and community members.
  • Strong oral and written communication skills.
  • Experience in crisis settings and appropriate emergency response.

Physical Demands / Working Conditions:

  • Work performed in office and community environments, requiring quick adjustment to changing priorities.
  • Tasks include standing, walking, reaching, climbing, balancing, twisting, leaning, moving from one location to another, stooping, kneeling, crouching, crawling, and sitting with keyboard use.
  • Travel by private and commercial transportation within the region may be required.
  • Work environment may be in recreational rooms or common areas with moderate noise levels.
  • Must be physically capable of climbing, standing, and lifting at least 10 pounds.
