Job summary

River Edge Behavioral Health in Macon, GA seeks an ICM Case Manager to provide paraprofessional social services across Bibb and Baldwin counties. You will participate in treatment planning, deliver crisis intervention when needed, and assist clients and families facing personal and social challenges.

The role emphasizes supportive counseling, social services liaison work, and coordinating case management activities to promote well-being within the program.

Qualifications

• Bachelor’s degree in human services or a related field is required.
• Minimum of two years of case management experience.
• Two professional references from former employers.
• Successful completion of background clearance and pre-employment drug screening.

Responsibilities

• Provide a broad range of paraprofessional social services to patients/clients and their families.
• Participate in treatment team planning and perform crisis intervention.
• Assist individuals in addressing personal and social challenges.
• Provide supportive counseling to consumers and families and/or serve as a liaison for social services.
• Perform case management duties, coordinating services and resources as needed.
• Facilitate group skills training to support client growth and development.
