Premier Health Partners, PC is a patient-centered gastroenterology provider in Buffalo, NY. We seek an LPN Navigator to support the care team with triage, education, and care coordination for IBD patients.

The role involves collaboration with the RN Navigator and GI providers, documentation in the EHR, and managing care plans for complex therapies. NY LPN license and current BLS certification are required.

Formación

• Current BLS certification and NY LPN licensure are required.
• Graduate of an accredited practical nursing program.
• Experience in ambulatory care, care coordination, or related clinical settings preferred.

Responsabilidades

• Collaborate with the RN Navigator and GI providers to coordinate care for IBD patients.
• Outreach and follow-up regarding treatment plans, labs, and appointments.
• Monitor testing completion and assist with care plan components.
• Educate on medications, adherence, and follow-up instructions.
• Support pre-biologic workflows including TB and hepatitis testing.
