Job summary

University of Washington is seeking an experienced Registered Nurse Care Manager for the Digestive Health Center at UW Medical Center-Montlake in Seattle. You will support patients with inflammatory bowel disease through care coordination and patient-centered planning.

The role requires 2 years of medical-surgical or digestive health nursing experience, an active WA RN license, and opportunities for occasional telework after probation. Day shift, full time, 40 hours per week.

Qualifications

• 2 years of recent medical-surgical or digestive health nursing experience.
• Care management experience preferred.
• Current licensure to practice as a Registered Nurse in Washington State.

Responsibilities

• Provides episodic and ongoing care management for a panel of patients with complex needs.
• Partners with patients to formulate, coordinate, and revise an individualized plan of care.
• Collaborates with providers and staff across the UW Medicine system to ensure seamless transitions of care.
• Performs clinical triage and monitors laboratory, imaging, and procedural results.
• Manages medication regimens and coordinates prior authorizations to ensure timely access to treatment.
