Job description

Under the guidance of the IBCCP Case Manager, performs intake, follow-up with program clients and providers to ensure that clients are accessing breast and cervical services, and provides support to clients. Provides clerical assistance to ensure the IBCCP goals are achieved. Performs other tasks as assigned to assist in IBCCP and the VNA Health Center.

Reports To: Director of Health Center Operations


Primary Functions:

  • Ensures IBCCP takes forms and other required documents are completed appropriately, and are error free, per IBCCP policies and procedures.
  • Communicates effectively with patients, families, Providers, VNA and hospital staff.
  • Notifies the IBCCP Case Manager of problems or alterations in patient status.
  • Takes responsibility for the review and implementation of all IBCCP guidelines and procedures as directed by the IBCCP program.
  • Completes applicable logs for women’s wellness tests completed.
  • Tracks women’s wellness patients. This includes managing medical records, making phone calls, sending letters and interpreting.
  • Processes IBCCP referrals, coordinates with patient to schedule their appointments and documents the referral progress in the EHR.
  • Organize and coordinate the flow of patient related documentation in the EHR.
  • Proficient in the use of EHR and IBCCCP related documentation in the EHR.
  • Proficient in the use of scheduling software and in the ability to schedule procedures efficiently and effectively.
  • Proficient in the use of Cornerstone.
  • Able to scan patient related documents.
  • Able to perform duties of the Mammography Secretary.
  • Attends IBCCP required meetings in Chicago and Springfield.
  • Assist the Mammography Technician with Mammography administrative assistant duties as needed.
  • Ability to work off-site clinics as assigned.
  • Understands and practices exposure control procedures.
  • Familiarizes and complies with all VNA policies and procedures.
  • Meets standards of behavior expectations.
  • Follows established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer immediately. Failure to comply with HIPAA policies and procedures will result in disciplinary action, up to and including termination of employment.
  • Other duties as assigned.

Compensation :Hourly Range: $20.00 - $21.45, depending on experience.

Shift Differential: $3.00 an hour for hours worked after 5:00 pm and any hours on a Saturday or Sunday.

Incentives : Earn up to $500.00 quarterly based upon meeting productivity goals.

Qualifications

  • Requires high school diploma or G.E.D.
  • 1-2 years experience in a medical office setting or applicable experience.
  • Effective communication skills, including neat and legible handwriting/printing.
  • Excellent computer skills with proficiency in Microsoft applications, including Word and Excel.
  • Health policies and requirements (See Personnel Policies).
  • Bi-lingual in Spanish/English as follows: Reading - Preferred. Written – Preferred. Speaking – Required.

Essential Functions/Physical Requirements:

1. Ability to communicate effectively verbally and in writing.
2. Tactile, visual and olfactory senses intact.
3. Manual dexterity sufficient to operate a computer.
4. Ability to deal effectively with high levels of stress.
5. Ability to frequently lift up to 50 lbs.
