Under the guidance of the IBCCP Case Manager, performs intake, follow-up with program clients and providers to ensure that clients are accessing breast and cervical services, and provides support to clients. Provides clerical assistance to ensure the IBCCP goals are achieved. Performs other tasks as assigned to assist in IBCCP and the VNA Health Center.

Reports To: Director of Health Center Operations
Primary Functions:
Compensation :Hourly Range: $20.00 - $21.45,depending on experience.
Shift Differential: $3.00an hourfor hours worked after5:00 pmand any hours on a Saturday or Sunday.
Incentives : Earn up to $500.00 quarterlybased upon meeting productivity goals.

Qualifications
1. Ability to communicate effectively verbally and in writing.
2. Tactile, visual and olfactory senses intact.
3. Manual dexterity sufficient to operate a computer.
4. Ability to deal effectively with high levels of stress.
5. Ability to frequently lift up to 50 lbs.
