Job summary

Indiana Professional Management Group is seeking a Wrap Facilitator for a full-time hybrid role serving Miami County, Indiana. The position combines remote work from a home office with occasional in-person community meetings within the county.

You will help families build wraparound teams, develop plans based on strengths and needs, and connect them to necessary services. Travel within the county is required.

Qualifications

• BA/BS degree and at least 24 months of experience in clinical, case management, skills building, child welfare, juvenile justice or K-12 education.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Home office with computer, internet, and smartphone.
• Willingness to travel within Miami County, Indiana for meetings.

Responsibilities

• Help families develop their wraparound team with professionals, friends, and family.
• Create a wraparound plan based on family strengths and needs.
• Help families access services and supports to meet goals.
• Facilitate regular community-based wraparound team meetings to review the plan.
