Qualifications

• A BA/BS degree is required and relevant experience preferred.
• Must be able to travel within Pike County and meet in person as needed.
• Background checks and valid driver's license required.

Responsibilities

• Create wraparound plans with families and a team of professionals.
• Facilitate regular wraparound team meetings and review plans of care.
• Help families access services and supports to meet goals.
