Job summary

You will develop personalized wellness plans, collaborate with providers, and facilitate wellness programs while supporting patient engagement and scheduling needs. A NBHWC certification and 2+ years of coaching experience are required.

Qualifications

Bachelor’s degree in health-related field and NBHWC certification required.
Minimum of 2 years of health coaching experience (one-on-one and group).
Experience with health promotion and behavior change methodologies.
Excellent communication and time management skills.

Responsibilities

Conduct individual and group coaching sessions to promote behavior change and chronic condition management.
Develop personalized wellness plans with clients and track progress.
Collaborate with healthcare providers for chronic condition coaching per guidelines.
Assist patients in scheduling medical appointments and referrals.
Facilitate wellness programs, group challenges, and biometric screenings.
