Job summary

Marathon Health is seeking a Certified Health Coach to join our integrated health center team in a part-time role. You will coach individuals and groups, build personalized wellness plans, and collaborate with providers to support chronic condition management.

The role includes coordinating appointments and facilitating wellness programs within a multidisciplinary team. Strong NBHWC certification and 2+ years of coaching experience are required, with proficiency in EMR systems and Microsoft 365.

Qualifications

Bachelor’s degree in health science, counseling, psychology, nutrition, social work or related field.
Master’s degree preferred.
Certification from NBHWC is required.

Responsibilities

Conduct individual and group coaching sessions to promote behavior change and wellness.
Develop personalized wellness plans with clients and track progress.
Collaborate with healthcare providers on chronic condition coaching per guidelines.
Assist patients in scheduling appointments and referrals.
Facilitate wellness programs and biometric screenings with the care team.
