Job summary

Primary Health Care, Inc (PHC) in Des Moines, IA, is seeking a Behavioral Health Practitioner to collaborate with a multidisciplinary team and provide assessment, brief and long-term therapy for patients across ages in a primary care setting.

The role supports in-person and telehealth services, with a preference for bilingual Spanish-English candidates and experience with pediatric therapy. Licensure through Iowa is required, and a strong emphasis on teamwork and regulatory compliance.

Qualifications

• Licensure as LISW/LMSW, LMHC or LMFT recognized by Iowa (temporary LMHC/LMFT licenses not acceptable).
• Master’s degree in Social Work, Counseling, or related field for licensure.
• Experience with third-party payers and related regulations.
• BLS for Healthcare Providers to be obtained within 90 days.
• Mandatory Reporter Training for Child/ Dependent Adult Abuse in Iowa.

Responsibilities

• Provide behavioral health care in PHC clinics, collaborating with healthcare teams to improve patient outcomes.
• Deliver brief and long-term therapy to individuals and families, including telehealth.
• Accept referrals and provide timely feedback to referring providers.
• Refer to community resources and coordinate specialty referrals.
• Maintain documentation and manage relevant databases.
• Provide clinical consultation and educational programs on behavioral health topics.
• Ensure compliance with regulatory requirements and PHC policies.
