MaineHealth is seeking a Social Worker (LSW) to join Lend A Hand within our population health management program. The role provides complex care management in primary care settings, addressing social drivers of health and coordinating with care teams to overcome barriers to health goals.

The position offers a hybrid work model (up to 2 days remote after training), a Maine State LSW license, and opportunities for professional development and benefits. Full-time, day shift, 40 hours per week.

Qualifications

• Bachelor's in Social Work from CSWE-accredited program required.
• Active Maine State Board of Social Work license required.
• Experience providing therapeutic services to adults and children with chronic issues.

Responsibilities
