Job summary

Humana Inc. seeks a Social Worker for its HRPM program to assess psychosocial needs and coordinate care for the top 5% of high-risk patients. This hybrid role requires in-clinic presence 2–3 days per week with remaining days telework, focusing on removing barriers to care and safe transitions.

You will lead assessments, coordinate with Care Coach and PCP, and connect patients to community resources, behavioral health services, and housing or financial supports as needed.

Qualifications

• MSW from an accredited program.
• Bilingual in English and Spanish with fluent reading/writing/speaking abilities.
• 3+ years of clinical social work experience with high-risk patients.

Responsibilities

• Provide psychosocial assessment, care coordination, and social needs interventions.
• Partner with Care Coach and PCP to address barriers and support integrated care planning.
• Conduct time-limited, goal-oriented social work interventions and resource navigation.
