Job summary

Golden Gate Regional Center (GGRC) in San Francisco County is seeking a Social Worker to assess needs and develop Individual Program Plans for developmentally disabled individuals, coordinating services and advocating for client rights. The role is hybrid, with home visits, GGRC's SF office, and remote work options.

The position requires knowledge of counseling, interviewing, and community resources, plus a commitment to person-centered planning and accurate documentation.

Qualifications

• Bachelor's Degree (from an accredited school of social welfare or related field preferred).
• Demonstrated interest in working with persons with developmental special needs.
• Access to reliable transportation.

Responsibilities

• Assess needs and develop Individual Program Plans for individuals supported.
• Coordinate services and manage a wide range of active cases.
• Meet individuals at residence or in the community to design and implement the plan.
• Initiate regional center funding and ensure services are provided; prepare required reports.
