Job summary

Arab American Family Support Center (AAFSC) is hiring a Solution-Based Casework (SBC) Case Planner for a full-time, hybrid role in the Bronx. The Case Planner will provide high-quality case planning, case work, and case management to at-risk families referred by ACS, using SBC foundations to promote safety and family stability.

Responsibilities include conducting home/office visits, coordinating with ACS, and ensuring timely documentation in Apricot and other systems.

Qualifications

• Bachelor’s degree from an accredited college or university required.
• Familiarity with PROMIS and Connections.
• Proficient with Microsoft Office and strong writing skills.
• Bilingual or multilingual fluency (Arabic, Bangla/Bengali, Hindi, Spanish or Urdu) preferred.

Responsibilities

• Conduct home visits and office visits to assess child safety and risk elements, complying with ACS requirements.
• Provide case work/case management services to families receiving Preventive Services.
• Evaluate family functioning and develop outcomes & service plans with families.
• Complete intake documents and maintain accurate case files and databases.
