Trinity Health in Troy, NY is seeking a registered nurse for a full-time, hybrid position located at 2231 Burdett Ave. The role features in-office days on Tuesday and Wednesday with remote work on Monday, Thursday and Friday.

The RN will use the nursing process to diagnose and treat patient needs, document in EMR, manage meds, coordinate referrals, and educate patients while maintaining safety and quality in a collaborative environment.

Competenze

• Associates or Bachelor's degree in Nursing preferred
• HS Diploma/equivalent required
• Current NYS RN license
• Basic Life Support certification
• 6 months previous RN experience
• Must be able to lift 20 lbs

Mansioni

• Take vital signs and document in EMR.
• Review and update medication list in EMR.
• Complete referrals and track patient adherence.
• Review prescriptions electronically and submit to providers.
• Obtain patient consent for procedures.
• Perform pre-visit planning and review quality metrics.
• Educate patients regarding medication and home care.
• Maintain clean and safe work environment and equipment.
• Maintain medical and drug cabinets in conjunction with team.
• Communicate via EMR and participate in daily huddles.
