Job summary

Qualifications

Responsibilities

• Education - Identifies learning needs and attends education programs; uses patient education materials.
• Evaluation of Care - Identifies outcomes and revises plan of care with interdisciplinary team.
• Implementation - Provides care for patients with complex problems; follows policies; prioritizes care; coordinates with resources.
• Leadership - Develops goals for professional growth and promotes interdepartmental collaboration.
• Patient Assessment - Synthesizes data to determine priorities; anticipates discharge needs; builds family relationships for advocacy.
• Planning - Collaborates with patient/family to update the plan of care; utilizes multidisciplinary resources.
• Research - Reads journals and brings ideas for assessment of applicability.
