Qualifications

• RN with 3–5 years in an acute care hospital required.
• Associate in Nursing or HIM is required; Bachelor’s in nursing preferred.
• RHIT or ADN; IL RN license preferred.

Responsibilities

• Review medical records for accuracy, completeness, and specificity of clinical documentation.
• Identify and clarify missing, conflicting, or nonspecific physicians’ documentation through queries and follow-up.
• Collaborate with clinicians to ensure accurate documentation of diagnoses and treatments and educate staff on best practices.
• Develop and implement effective queries to resolve documentation issues and track outstanding responses.
