Job summary

Mass General Brigham Community Physicians, Inc. is seeking a skilled RN Case Manager to coordinate and support healthcare services within the facility and with external referrals.

You will focus on developing and coordinating community service plans with families and patients to ensure seamless care. The role emphasizes outreach, enrollment activities to meet program eligibility, and collaboration with service providers and medical staff to optimize patient outcomes.

Qualifications

• Associate’s Degree in Nursing is required.
• Active RN Licensure in Massachusetts required.
• 2+ years of case management and/or discharge planning required.
• Experience with complex dually eligible populations preferred.
• Knowledge of Managed Care preferred.
• Understanding of diagnostic criteria for dual conditions and placement criteria within the continuum of care preferred.

Responsibilities

• Conduct client assessments and planning.
• Modify patient treatment plans as indicated by patients’ responses and conditions.
• Prepare patient/family for discharge.
• Communicate with third party payers to obtain necessary authorization for reimbursement of services.
• Define care goals with education, information, and directions for patients and families.
• Review cases with medical directors on challenging cases as needed.
• Maintain accurate, detailed reports and records.
