Job summary

North Shore Community Health (NSCH) is seeking a Primary Care Team Nurse Care Manager (RN CM) to coordinate care for NSCH's most vulnerable patients. You will work with the primary care team, specialty NSCH care teams, and community providers to optimize health care utilization and outcomes.
