Job summary

NorthKey Community Care is seeking a full-time Integrated Care Coordinator RN to engage with behavioral healthcare clients, ensuring they receive the most effective care for their behavioral health and primary care needs. This hybrid role rotates between clinics and home-based care with typical hours 8:30 to 5, Monday through Friday.

Ideal candidates hold an active RN license in Kentucky, strong communication skills, and experience in behavioral health.

Qualifications

• Active RN license in Kentucky.
• Excellent written and verbal communication skills.
• Psych nursing experience preferred but not required.

Responsibilities

• Engage clients with SMI/SED/SUD in structured, collaborative care.
• Promote independence and maintain well-being, functioning.
• Provide evidence-based care for complex behavioral health cases.
• Identify and address gaps in care to prevent hospitalizations or loss of independence.
• Provide education to improve client functioning and self-management.
• Complete prior authorizations as needed.
• Participate in staff meetings and coordinate referrals.
