Job summary

Qualifications

Responsibilities

• Conduct comprehensive nursing assessments for physical and behavioral health needs.
• Administer and monitor psychiatric medications, including injections; monitor effects.
• Educate clients and families about medications and treatment options.
• Coordinate referrals and care transitions across settings with the care team.
• Maintain documentation in the EHR and track referral statuses.
